Gemstones Take-in Form and Service Agreement

The purpose of this form is to provide our customers a way to know and identify their diamonds before they can leave it with us and to verify it back at the time of pick them up. Customer agrees that the description of the gemstone herein represented is correct. Hannon will identify, plot on this form and show the customer major identifying characteristics only ( internal or external) which provide positive identification of this gemstone. Hannon Fine Jewelry shall not be responsible for the actual or full identification or condition of the gemstone at the time of receipt; that gems - after cleaning and close inspection - may reveal damages on the gem or on its mountings not seen before. The reason for this is that mountings limits the full and accurate observation of measurements and evaluation of quality, condition or treatments on gemstones. Therefore, only provisional and limited grades may be assigned.
